In consideration of student’s ability to participate in the activities, the undersigned, for the parents and for student, waives any and all claims the parents and/or the student may have against Master’s Challenge Academy, the directors/teachers, other students and Francis Asbury United Methodist Church for any injury or damages arising out of, or in connection with, student’s participation. This consent shall be a continuing consent for the summer camps beginning May 8, 2023 to June 30, 2023. This consent may be revoked only in writing signed by the natural guardian.
